New Patient Registration
If you wish to register as a patient please come into the surgery to collect your registrations forms. We will need to confirm that you are living in the Folkestone area and within our practice boundary. You will be offered a new patient check with one of the nursing team. You will need your previous GP details and your NHS number if you know it.
If you require medication and you have just registered with us you will need to book an appointment with a GP so these can be added to your computer records at the clinic.
Under the terms of the current General Medical Services contract that we hold with government all of our patients are registered under Dr M Fernandes as the Lead GP. We have an excellent clinical team here, you can choose to see any of our GPs. Please ask to see your usual GP for your continuity of care.
